311東日本大地震是人類史上罕見的複合型災害，地震、海嘯、核災同時來，福島縣知事內堀雅雄說，311東日本大震災最常聽見的是一句話是「預想外」，故今後制定防災守則，一定要設想最壞的情況，不要讓「預想外」這個名詞再度出現。
內堀3月初在東京召開的記者會上指出，福島核災不僅是進行式，同時也是未來式，福島縣至今仍有約10萬人流落在外，過著避難生活。所幸指定避難區多以解除，福島縣內除了最後7％的指定避難區無法居住外，其他地區已恢復正常。
福島縣災後特別加強避難區、避難路線的設定及災害管理，且平時便與外縣市簽訂合作協議，以備不時之需。岩手縣的重災區大槌町，當時海嘯時，最高水位達22.2公尺，死亡和失蹤人數占人口的1成，達1285人，約6成的房屋受損，使災後重建工作延宕。
大槌町町長平野公三3日受訪時表示，從311得到的最大教訓是，過於相信硬體使危機意識降低。岩手縣在昭和、明治時代都經歷過海嘯，因此在沿岸建了很高的防波堤，人們確信堤防可圍堵海嘯而掉以輕心，結果造成慘重的傷亡。大槌町的防波堤工程將從原來的6.5公尺加高到14.6公尺。
相較於大槌町，宮古市是重建速度較快的地方自治體。宮古市長山本正德4日受訪時指出，1933年3月3日曾發生芮氏規模8.5級的昭和三陸大地震，當時宮古市死亡和失蹤人數高達911人。因此宮古市每年3月3日一定會舉行防災演習，訓練市民逃到高台上，此例行演習在311震災時奏效，但仍有181人喪生，原因還是出在「預想外」。
16公尺高的無情海嘯竟翻過號稱「萬里長城」，總長2433公尺、高10.65公尺，呈X字型的3道防波堤，這是當地民眾萬萬想不到的。山本表示，不過，防波堤還是保護了部分地區，故仍將修建防波堤，建設費由中央政府出，縣政府負責管理維護。同時也移山建高台，加高地盤的工程已經完工，民眾也開始迎接新的生活。
談到311震災的教訓時，山本表示，這次有消防救災人員為疏散民眾而犧牲，故已重新檢討避難方式，將要求引導避難的消防隊員也要為自己留10分鐘的避難時間。
